Lobar Pulmonary Transplantation

چکیده

There is limited experience in lung transplantation. The indications of lung transplantation in children are different compared with adults. There are some special problems in pediatric lung transplantation such as donor shortage, size discrepancy, growth potential of transplanted lung and hemodynamic problems. Although some of these problems have been solved, only 2% of lung and hearth-lung transplantation has been done in pediatric patients. Living related organ donation and lobar/segmental pulmonary transplantation may be options for donor shortage and size discrepancy and this area is the new and very promising part of pediatric lung transplantation. Contralateral pulmonary artery banding after single lobar lung transplantation.

A 14-year-old female patient underwent right single living-donor lobar lung transplantation for bronchiolitis obliterans after bone marrow transplantation. The patient experienced a complication with severe hypoxemia requiring venovenous extracorporeal membrane oxygenation, which appeared to result from significant ventilation-perfusion mismatch caused by preferential ventilation of the transpl...

Pulmonary arterioplasty for the remaining arterial stump of the donor and the arterial cuff of the donor graft in living-donor lobar lung transplantation.

In living-donor lobar lung transplantation (LDLLT), donor surgeries are conducted in ways that ensure proper dissections for both donors and recipients. We report a case of LDLLT, in which pulmonary arterioplasties with autopericardial patch were performed on both a donor and a recipient. Since excision of the lingular branch of the pulmonary artery was carried out far lower than that of the up...

Two case reports of successful withdrawal of mycofenolate mofetil after living donor lobar lung transplantation.

BACKGROUND Lung transplantation cases have immunosuppression maintained using a calcineurin inhibitor, anti-metabolites, and steroid. CASE REPORT We report 2 clinical cases in which anti-metabolites (mycophenolate mofetil) were successfully withdrawn after living donor lobar lung transplantation by monitoring immune function using the ImmuKnow® assay. In the first case, a 43-year-old woman un...

A decade of living lobar lung transplantation: perioperative complications after 253 donor lobectomies.

Living lobar lung transplantation places two donors at risk for each recipient. We examined the perioperative outcomes associated with the 253 donor lobectomies performed at our institution during our first decade of living lobar lung transplantation. There have been no perioperative or long-term deaths. 80.2% of donors (n = 203) had no perioperative complications, while fifty (19.8%) had one o...
